Describe the pathway and denominator
Investigate the pathway from initial information through diagnosis, treatment and follow-up. For each step, define who needed the service, who reached it and who completed it. Studying only people served may hide those excluded before any record was created.
Measure conditions and consequences
Waiting time, distance, cost, availability and communication can measure access. Compare groups while considering record quality and social conditions. Observed differences do not justify assigning biological causes to social categories or blaming individuals for structural barriers.
Test a verifiable improvement
A navigation or reorganisation proposal must identify the barrier targeted and how it will be evaluated. Measure reach, completion and potentially unequal effects. Include people’s experiences and avoid declaring success solely from a higher total number of visits.
Interpretation limits
Aggregate indicators may hide differences within the same area or group.
These concepts guide health services research; they do not classify individual clinical risk.
